Diabetes Program

The SCAHC Diabetes Program has 3 different teams:

  • Clinic
  • Outreach
  • Fitness

 

Diabetes Clinic

The clinical team has Pharmacists, Registered Dietitians, Nurses, and Medical Assistants that will help you better manage your diabetes. We provide blood sugar monitors, diabetes education, and one-on-one case management. We also provide diabetes foot exams during your visit.

 

 

 

 

 

 

 

 

 

The Diabetes Program is located in the Specialty Clinic.  If you would like to make an appointment with Diabetes Program call (928) 475-1467. You may need to see a provider before scheduling with us.


Fitness Centers

The services offered by the San Carlos and Bylas Fitness Centers are group exercise,  personal training, monthly screenings, and cardio & strength equipment. Our friendly fitness instructors are ready to assist you with proper form and technique in lifting weights and using different workout equipment. They can help you reach your fitness goals.

The Bylas Fitness Center is located West of the Clarence Wesley Health Clinic. Business hours: Monday – Friday from 5:00 am to 8:00 am and 4 pm to 8:00 pm. Saturdays 6:00 am to 10:00 am. Phone: (928) 475-2319.

The San Carlos Fitness Center is located behind the Department of Health and Human Services (DHHS) in downtown San Carlos. Business hours: Monday – Friday from 5:00 am to 9:00 pm,  Saturday from 8:00 am to noon. Phone: (928) 475-7973.

Community Outreach

The Outreach Team works to prevent diabetes. They provide community fitness activities, cultural events, and prevention education throughout the San Carlos communities.  If you would like to schedule a special event, please call (928) 475-1241.

If you scored 5 or more points, please call the Outpatient appointment desk at (928) 475-1499. Ask to schedule a medical provider appointment to be tested for diabetes.
